(3) (a) A firm, partnership or corporation desiring a certificate of
22authorization shall submit an application to the department on forms provided by
23the department, listing the names and addresses of all officers and directors, and all
24individuals in its employment licensed to practice professional geology, hydrology or
25soil science in this state who will be in responsible charge of professional geology,
1hydrology or soil science being practiced in this state through the firm, partnership
2or corporation and other relevant information required by the appropriate section of
